Community members have been complaining about their difficulty in obtaining the appropriate care as well as a lack of communication between their current health professionals. Write a proposal that includes the following:
Describe the current population.
Interpret the current operations of the community health centers. Note any potential ethical violations.
Document your proposed collaborative model and implementation plan, including a description of the health care professionals and their roles in the model.
Investigate potential limitations and barriers to implementation. Provide examples to support your findings.
Determine internal policies and examine potential external policies that are needed to make implementation a success.
