Kentucky Homeplace (Ruralhealthinfo.org) program was started in the United States to address lifestyle choices, substandard health insurance and environmental factors that have caused an array of avoidable diseases in rural Appalachian Kentucky. Appalachian Kentucky is one of the unhealthiest regions in the United States. This region has had rising cases of obesity, cancer, heart disease and diabetes. Kentucky residents lack the appropriate knowledge regarding health conditions due to cultural inhibitors, poor transport, and financial burdens (Blank, 2013).

The program has greatly brought about improvement in the health and education in more than 29 counties in the Appalachian region, Eastern Kentucky. Kentucky Homeplace trains people who were born in Kentucky as Community Health Workers (CHWs) in a bid to provide access to numerous health services to the population.

Moreover, Kentucky Homeplace has provided free health care services for more than a decade now to the entire population of Kentucky. The free health care services they provide include Eye exams, Diabetes Self-Management, dental services, health coaching for families and free health information.

Kentucky Homeplace over the years has kept the targeted population healthier. This is evidenced by their participation in care coordination and health coaching on chronic disease management, diabetes, preventive screenings and prenatal care. In the last decade, the program has served more than 150,000 clients and have provided over 4 million services with a medication service value of 300 million dollars.
