CVL and Arterial Lines
Discuss the most common complications of central venous catheters and arterial lines. Remember that different placement sites can be related to different types of complications. Make sure and include interventions to mitigate complications in your discussion, and remember to discuss both arterial line complications and central line complications.

Thoracentesis and Thoracostomy
Discuss at least 4 circumstances in which a tube thoracostomy (chest tube insertion) is necessary, in addition to determining the size of the tube required, and evaluation of the fluid drained. Describe the difference between transudate and exudate and why it is important, and finally, describe under what circumstances you would consider discontinuing the chest tube.

Evaluation of a lump

A 42-year-old woman presents for evaluation of a lump that she noticed in her right breast on self-examination. She says that she does not examine herself often, but that she thinks that this lump is new. She has not had any nipple discharge and has no breast pain, although the lump is mildly tender on palpation. She has never noticed any masses before and has never had a mammogram. She has no history of breast diseases and has never had a biopsy. There is no history of breast cancer in the family. She takes oral contraceptive pills regularly, but no other medications. She does not smoke cigarettes or drink alcohol. She has never been pregnant. On examination, she is a well-appearing, but somewhat anxious, thin woman. Her vital signs are within normal limits. Her general physical examination is normal. Examination of her breasts reveals no skin dimpling or retraction and no nipple discharge. In the lower outer quadrant of the right breast there is a 2-cm, firm, well-circumscribed, movable mass that is mildly tender. No other masses are felt, but the breast tissue is noted to be firm and glandular throughout. No axillary, supraclavicular, or cervical lymphadenopathy is appreciated.

70 year old male brought in by family with lethargy, loss of appetite, and “sleeping a lot.” His BP is 96/50, pulse 97, respiratory rate 28, pulse ox 90% on RA. BUN is 24.
45 year old male with 40 pack year hx smoking, complaining of having a hard time catching his breath. BP is 142/92, pulse 84, RR 34, pulse ox 88% on 2L. BUN 16.

HK is an 83 young woman complaining of pruritic “hives” (small red bumps) one day ago on trunk and thighs accompanied by nausea and lightheadedness. Her left eyelid also became swollen. Symptoms occurred at rest and lasted two hours.
Unfortunately, we had no camera in the office during her initial. The swelling of her eyelid lasted a bit longer than the bumps on her trunk and thighs. Her lightheadedness represented presyncope.

Cardiac: regular rate in 80s, normal exam
Skin: patch of erythema on back and trunk beneath bra strap, erythema over beltline, red papules on lower anterior chest, ecchymoses on shoulders and legs. The rash seen on exam is different than the transient rash that she had one day ago.
