Specific Facility Change: Q 15 Minute Rounding

My current role at my organization is a psychiatric nurse specializing in the adult population suffering from mental illness as well as chemical dependency. The majority of our patients treated on my unit suffer from depression, suicidal thoughts and urges to self-harm. In 2015, death by suicide was the 10th leading cause of death in the United States, resulting in over 44,000 deaths & approximately 1.4 million suicide attempts (Centers for Disease Control and Prevention, 2017). Due to the high rates of death by suicide, Banner Behavioral Health Care implemented Q 15 minute rounds to provide patientís a safer level of care while being treated inpatient at my organization. Prior to this change in policy, we were doing Q 30-minute patient rounding.

Impact of Q 15 Minute Rounding on Roles and Responsibilities

When implementing this policy change at my organization, it primarily impacted the role of the behavioral health specialist that was responsible for doing the Q 15-minute rounds on patients. This policy change was met with some resistance, it is common when implementing new policies that staff feel stressed and discontent at first until they fully adapt to the change in policy (Brown, Wey & Foland, 2018). Prior to the implementation of the Q 15-minute patient rounds, the behavioral health specialist felt like it would consume too much of their time trying to check on their assigned patients that often and continue to meet their other job requirements. It was obvious that staff were frustrated and needed to be reminded that we are all a team and that our primary focus is to maintain patient safety. Management was responsible for meeting with staff and educating the organization on the policy change to help us adapt to the change.

The rationale for Q 15 Minute Rounding & Intended Outcomes

The rationale behind the change from Q 30-minute rounding to Q 15-minute rounding was to maintain patient safety in the inpatient behavioral health setting. Statistics have shown that up to 20% of suicide attempts occurred in-between staff rounding on patients. By reducing the time from 30 minutes to 15-minute intervals has proven to reduce self-harm in the hospital setting as well as suicide attempts (Flynn et al, 2017). The intended outcome of implementing Q 15-minute patient rounding was to maintain patient safety. Case Study:

Increasing incidence of staph infections in one community led to a prevalence study. It was discovered that the prevalence of community acquired staph infections had doubled in this particular town. It was two visiting nurses who launched the initial investigation. They had noticed that their own caseloads had more people with staph infections being treated, who had not been recently hospitalized. They began tracking the incidence and found a trend. They followed up by creating a survey that asked questions about the activities prior to the infection. Also they did an Eco-map for each person.

They noticed that all of the people did have two things in common. They all had a relative who worked in health care in some way and all belonged to the same local ethnic affiliated club. The nurses took their findings to the local health department. The health department did a more in-depth investigation which uncovered the finding that all of the people had contact with two nurses aides who worked at a local nursing home. It was noted that the nursing home unit where they worked had multiple cases of staph infected wounds. The home had also begun a program of training certain aides to do dressing changes. Both of these aides where in that group. Once discovered that practice was stopped, and an education program on infection control was implemented in the nursing home and throughout the community.

The following two paragraphs are two classmates responses to chapter 7 I need to respond to these two post giving my own input about the chapter.

1) So from reading chapter 7, one of the more interesting points I learned from the chapter is that while it is highly unlikely, it is possible to transmit HIV through saliva.

This is something that previously I had always learned was not possible, but according to one of the figures in our book. transmission through saliva is possible, it is just considered a low risk transmission though.

This is most likely because it is believed that if someone has a sore or cut inside their mouth, can transfer infected blood to another individual with an open cut. This is the same concept dealing with contracting the virus through oral sex, the probability is unlikely as the virus needs to enter into the uninfected person’s bloodstream which usually will require a an open cut or sore.

Here is another website I looked up as secondary research into it, this is from the CDC and covers a lot of different types of transmission possibilities. It also notes methods that the virus cannot transmit by, such as through mosquito bites or social contact.

I knew that having sex can lead to HIV but I also didn’t know that the first time you have sex with someone with HIV doesn’t mean that you’re going to get it or it’s going to show right away also growing up I always had a question about can you give your child HIV while giving birth well learning in class and reading in this chapter that you can transmit HIV to your child at birth I found that very interesting.

Also it was very interesting to me that Contact between broken skin, wounds, or mucous membranes and HIV infected blood or blood contaminated by body fluids can lead to HIV.

I didn’t know that Deep open mouth kissing from both partners that have sores or bleeding gums and blood from a HIV positive partner gets into the bloodstream of the person who doesn’t have HIV. HIV Is spread through saliva according to the book but I’ve always heard that someones saliva cannot give you HIV necessarily.

After reading this chapter I realized that I am not very knowledgeable of HIV. I’m sure after reading more chapters and doing more assignments and paid attention to the professor I will learn more about HIV and all my questions that I may have will be answered.
