You’ve just left an all-hands meeting at your company*. The CEO was very upset at the rise of shadow IT projects – a major indicator that the company’s internal information system has failed to meet its needs.

Because the current information system is inadequate, inefficient, and outdated, the CEO is inviting everyone in the organization to propose a new operational, decision support, or enterprise information system to replace it. The executives have allocated $5 million to fund the most promising idea.

This is your chance to make a difference in the company (not to mention your own career). Write your proposal as a memo that the entire C-suite will review. Include at least these points, in your own words, to be persuasive:

  1. Identify the main functions of your proposed information system and why they are important to the business.
  1. Describe what types of data your information system will hold and how data quality will be ensured.
  1. Explain how the old information system handles the functions you mentioned, the problems that occur, and why your information system will handle things better.
  1. Offer evidence of feasibility: Show that similar information systems have been built successfully and that they save more money than they cost.

The executives are busy, so keep your memo to 1-4 pages and avoid any extraneous content.

*You may use a current or former employer, but do not disclose anything confidential. Or, you can pick another organization if you are familiar with their internal (not customer-facing) information systems. You can disguise the organization and populate it with famous names. Made-up companies are problematic because of the amount of detail and realism they require.

Hector, age 19, is a student at Mid-city Community College (MCC), the largest community college in the state. He is there on a scholarship for soccer. The counseling center at MCC referred Hector to you. He was initially seeing a counselor at the school because of adjustment and academic problems. Hector also tested positive for cannabis in a required drug screen by the athletic department. The soccer coach considers Hector, a star player. The coach wants him “fixed” and to receive passing grades in his classes so that he is eligible to play.

Hector considers himself of multiracial background; his father is Mexican-American, and his mother is Caucasian. They met while serving in the first Gulf War. Hector’s father, Julian, has struggled since the war due to Gulf War Illness, experiencing fatigue, headaches, skin rashes, and so on. Because of his medical issues, employment has been unsteadied, contributing to financial difficulties for the family. Carol, Hector’s mother, returned to school after her military service, received a degree in education and works as an elementary school teacher. The MCC counseling center described Carol as a “helicopter mom.” Even before Calvin, the outpatient center counselor met with Hector or completed confidentiality releases, Carol called and left three messages. Carol is overwhelmed with the care of her husband and their four children. Hector is the oldest. Hector’s only opportunity for a college education is to maintain his athletic scholarship. His high school grades were slightly below average. He was diagnosed with ADHD in elementary school; however, Hector states that he does not like how he feels on Ritalin. He says that marijuana is the only thing that calms him down. He is anxious to get Calvin to state that there is no substance-abuse problem and to return to playing soccer and living the rest of his life.

Rodney and Cassandra were having a drink in the pub. Rodney had been drinking beer since they arrived, but Cassandra was drinking lemonade because she was intending to drive home. Derrick and Raquel saw Rodney and Cassandra at the bar and approached them to say hello. Raquel wanted revenge for what Cassandra had done to her, so she punched her in the face and Cassandra fell awkwardly and cut her head open on the bar.

62 year old female with history of CHF was admitted from the Emergency Department with Left Lower Lobe Pneumonia.

  1. She has been living independently at home with no assistance needed with cares.
  2. She was complaining of pain with inspiration, vital signs were normal; oxygen saturation was 90% on room air.
  3. On hospital day 2, she was found on the floor at 23:45 in severe pain.
  4. X-rays revealed a hip fracture.
  5. The next day she was taken to surgery for repair.
  6. During surgery she had an MI and expired while on the table.
  7. This is a reportable event due to fall with serious injury.
  8. The patient had been incontinent soiling the bed and gown, but not floor.
  9. Using the information provided, identify root cause statements for this event
  10. The nurse came on at 2300 that evening and got report which indicated the patient was pleasant and cooperative, asking for help as needed, didn’t attempt to get up on her own, used the call light.
  11. Looking at the medical record found the patient had been given a sleeping pill and pain medication at 10pm, the same as the previous night and the sleeping pill is the same as she takes at home.
  12. At 23:30, the nurse checked the patient to do vital signs, found her dozing but easily arousable although a little confused when first awakened.
  13. The unit became busy as they just received a fresh surgical patient post operatively.
  14. At 23: 45, a nurse walking by the room heard a noise and went to investigate.
  15. She found the patient on the floor and called for help.
  16. The patient had been incontinent of bed and gown, the floor was dry.
  17. They had to move furniture in the room to get at patient as there were extra chairs in the room after her family visited that evening.
  18. Her oxygen tubing was wrapped around her IV pump.
  19. The physician came immediately and assessed the patient.
  20. X-rays were ordered and the patient was taken to x-ray.
  21. The x-rays revealed a hip fracture.
  22. Orthopedics saw the patient and scheduled her for surgery the next day.
  23. She was prepared for surgery and sent the next day.
  24. There was the usual staffing levels and mix of 8 patients to one nurse.
  25. The unit was busy but not chaotic.
  26. The staff knew there was a policy on risk assessment of falls.
  27. They indicated they had been educated and had their skills validated, however they didn’t think it was helpful and stated they would like more direction than what the policy provides.
  28. Risk of falls was assessed and documented on admission.
  29. The patient was found to be at very low risk with no issues.
  30. No reassessment was documented.
  31. Report communicated the patient was pleasant and cooperative, asking for help as needed, didn’t attempt to get up on her own.
  32. RT had seen the patient at 2200 and placed the patient on O2 at 2L for wheezing.
  33. He gave her extra long tubing as she liked to sit up in a chair.
  34. RT had documented their adding oxygen, but not the long tubing and did not verbally pass this information on to the nurse

Casey Anthony was indicted in October 2008 on a charge of the first-degree murder of her daughter Caylee Marie Anthony. In December 2008, a meter reader found the skeletal remains of Caylee near Casey’s home. Caylee was reported missing on July 15, 2008, a month after she disappeared, according to her mother Casey. In April 2009, the state attorney’s office said that the prosecutors planned to seek the death penalty for Casey. On July 5, 2011, Casey was found not guilty of first-degree murder after a thirty-three-day trial in Orlando, Florida. Casey was convicted of charges of misleading law enforcement and was released from jail on July 17, 2011. The verdict had been termed as travesty of justice and many people feel Casey got away with murder.

The Casey Anthony case was extensively covered by local media in Florida. When a case receives this amount of local coverage, it usually goes national but only during the trial. In this case, before the defendant had gone to trial, the case of Caylee’s murder had been on the national television talk show circuit, making the ratings for ABC’s 20/20, NBC’s Dateline, Fox’s America’s Most Wanted, Nancy Grace, Greta Van Susteren, and Geraldo Rivera.
